Registration
A parent/guardian or 18 year old student must complete the registration process prior to trying out for any sport during the school year. The registration process must also be completed prior to the first official day of practice.
FinalForms
The Berkley Schools Athletic Department has partnered with FinalForms, an online forms and data management service.
- FinalForms allows you to complete and sign athletic participation forms for your athletes.
- FinalForms saves data from season-to-season and year-to-year -- you will never need to enter the same information twice.
- FinalForms pre-populates information wherever possible, for each of your athletes, saving you time.
- You may review your data at any time to verify it is current.
- You will be required to sign your forms once per year and after any update.
- Annually, students must upload a current physical (completed on or after April 15th, 2026), and sign their forms.
- No paper copies of physicals will be accepted.
- When registering on FinalForms, please select all the sports you may be interested in participating.
